Please be advised that Medical Mutual has sent out Renewal applications to a portion of their insureds. These Reapplications are required to be completed approximately every three years. Even though you might think that this is for your 2021 renewal, it is really to make sure you are both covered and rated correctly, RIGHT NOW! Please complete and return them to your carrier or your agent by no later than April 22, 2021. If any additional questions arise, the carrier will send a follow-up notice to you requesting the additional information. All Reapplications must be cleared prior to the fall of 2021 to make sure the policy does not get Non-Renewed and to have renewal invoices released in a timely manner.
We encourage your to complete your Renewal Application online by visiting http://www.mmlis.com, signing on to “My Account,” and selecting “Reapplication” under the ” My Policy” tab. Once you have accessed the webpage, follow the on-screen instructions to complete the application.
